From time to time, it is or will be necessary for you to supply to the Company data such as your name, insurance account number, contact number, postal address, email address, HKID /passport number, birth date, gender, age, credit card number, bank account number, occupation, education, and personal interests etc.. Failure to supply such data may result in the Company's being unable to realize your preferences or provide its services to you.

Purposes for which data are used

If you do not allow and agree us to use the data for the voluntary purposes including direct marketing as listed above, please tick the opt-out checkbox when you fill in the Company’s forms and any other documents (if applicable).

    or technical services) in connection with services provided to you;

(3) Other nurses, doctors, dentists, TCM practitioners, pharmacists, or health care professionals, in the case
     of referrals from or to any of such professionals;

(4) Your employers and any insurers who are providing medical benefits to you (if necessary);

(5) Any debt collection agencies (if necessary);

(6) Any person or entity to whom we are obliged to make disclosure under the requirements of law; and

(7) Any government and judicial bodies or regulators.

Access and Correction of Personal Data

You have the right to:
1. check whether the Company holds any personal data relating to you and the right of access to such data;
2. require the Company to correct any data relating to you which is inaccurate; and
3. ascertain the Company's policies and practices in relation to personal data.
